Photo ID - Library

Accepted forms of ID:

• An Oregon State Driver’s License or Identification Card showing current address
• Any picture ID AND verification of current address. Address verification can be any official document that includes your name and current address, such as mail posted in the last 30 days, an imprinted check, a lease, a current rent receipt, or a utility bill.

Children’s library cards

• Children ages 15 years old and younger need the signature and ID of a parent or guardian to apply for a library card.
